Benefits of case management
Traumatic injury can hit anyone at anytime, and the effects can be devastating - physically, psychologically, and socially. Severe injury can lead to, for example, loss of function, decreased income, loss of status, and relationship problems.
Statutory services provide for basic needs, but usually do not have the facilities for a coordinated progressive assessment and rehabilitation package.
That's where case management can help - obtaining the best possible outcome for a claimant.
Evidence of past, present and future needs
In order to obtain the best outcome for a claimant you need to know
what they were like, how they are now, and how they may be in the future. Drawing on their experience and expertise the Case Manager will provide you with this information to help you cost a claim.
Achieving best outcome for client
The Case Manager’s role is to assess for current and future need. Using this as a baseline they will work with the claimant and appropriate therapists to increase potential, improve function, provide equipment, and aim to maximise independence.
Prediction of outcome
Predicting outcomes is not an exact science but, by using the evidence gained through the assessment and rehabilitation process, The Disabilities Trust Case Managers will provide evidence-based predictions of outcome.
